Faulty Trench Access Causes Construction Injuries

Trench Access Construction Fatality
Construction workers were installing pipe in a trench. Their employer had the workers enter and exit the trench by being hoisted in an excavator bucket. As the excavator operator was swinging the bucket out of the trench with the workers, the bucket jerked causing one of the workers to fall from height. He suffered a severe traumatic brain injury and later died from his head trauma.
Prevention of Similar Trench Construction Fatalities
To prevent construction site hazards of entering and exiting a trench, contractors must do the following:
- A safety plan must be created to eliminate or minimize construction hazards.
- Prohibit the use of buckets from transporting people.
- Workers must be instructed in the recognition and avoidance of unsafe conditions and the regulations applicable to their work environment to control or eliminate any hazards or other exposure to injuries (29 CFR 1926.21(b)(2)).
- An adequate means of exit and entrance must be provided for trenches four feet or more, i.e. ladders or steps (29 CFR 1926.652).
