Indiana Worker Dies in Construction Site Accident on a Metal Roof

Worker killed during the installation of a metal roof. The workers at the jobsite were not using fall protection equipment. Wind picked up a piece of the sheet metal. A worker falls from roof and hits a dumpster below with his head. He suffered a severe traumatic brain injury that caused his death.
In investigating the death, the Indiana State FACE investigators made the following recommendations:
- Fall protection equipment must be used when there is a chance for falls from height;
- Weather conditions must be suitable for the work being performed; and
- A competent person must make sure that the work place is safe for everyone on the jobsite.
