Indiana Workers are being Injured on Suspension Scaffolding when Safety Precautions are not taken

A worker is on a motorized suspension scaffold. The scaffold is suspended by two wire ropes - one on each side. The worker is not using any personal fall protection. One of the wire rope ends fails. The worker falls from height.
This is an example of how construction fatalities occur when proper precautions are not taken. In this case, the general contractor should have had the following:
- A competent person inspecting all wire ropes, slings, hangers, platforms and other supporting parts prior to installation and before every use (29 CFR 1926.451(i)(7)).
- A safety program and inspection program ensuring that everyone on the construction project use personal fall protection when on a suspension scaffold (29 CFR 1926.451(I)(8)).
- A competent person verifying that the suspension scaffold is properly installed and has lifelines securely attached to substantial members of the structure or rigging lines (not to the scaffold) (29 CFR 1926.451(I)(8)).
- The lifeline is being adjusted as the work progresses (29 CFR 1926.451(I)(8)).
