Roofer Falls from Roof to his Death
A roofer stepped back and tripped over the parapet. He fell to his death. The roof failed to have a safety monitoring system, warning lines or a motion stopping safety. There were also two unguarded duct openings on the roof.
Prevention Against Similar Construction Fatalities
- Temporary openings must have railings to prevent falls from height (29 CFR 1926.500(b)(7)).
- A safety monitoring system must be used on roof if the roof is a low pitched, fifty feet or less in width and no mechanical equipment is being used (29 CFR 1926.500(g)(1)(iii).
- Low-pitched roofs at heights greater than 16 feet must have warning lines around work areas (29 CFR 1926.500(g)(3)(i)).
- Workers must be instructed in the construction site hazards on a roof (29 CFR 1926.500(g)(6)(ii)(a) through (f)).
- All unprotected roof sides and edges must have a MSS system (29 CFR 1926.500(g)(5)).
