Workers were sandblasting on a two-point suspension scaffold at heights. The scaffold attachments to the cable failed. The cable released and the scaffold crashed to ground. The workers were not tied off.
Prevention of Other Similar Scaffold Accidents
- Contractors must instruct workers on the recognition and avoidance of construction site hazards (29 CFR 1926.21(b)(2)).
- Workers must be provided and use lifelines attached to substantial members of the structure and not to the scaffold for protection against construction fatalities (29 CFR 1926.451(i)(8)).
- Scaffolds must use tie backs as secondary anchorage on two-point suspension scaffolds (29 CFR 1926.451(i)(4)).
- A competent person must conduct periodic inspections before and while the scaffold is in use (29 CFR 1926.451(i)(7)).
- Scaffolds must have attachment components capable of sustaining four times the maximum load (29 CFR 1926.451(i)(2)).
