Iron Worker Falls from Height When Steel Columns and Bar Joists Collapse

Iron Worker Fatally Injured in Steel Structure Collapse
Iron workers were installing bar joists on a building under construction. The bar joists were supported by steel columns. The columns were not framed in two directions, and the bar joists were not bolted to the columns.
The bar joists shifted. The columns leaned. The bar joists and columns collapsed. Two iron workers rode the iron down. One was fatally injured, and the other received serious injuries.
Prevention of Construction Fatalities from Similar Falls from Height
- The structural steel fabricator and erector contractors and general contractors must ensure that during steel framing where bar joists are being used and columns are not being framed in two directions with structural steel members, the bar joists shall be field bolted to provide lateral stability during construction (29 CFR 1926.751(c)(1)).
- The structural steel fabricator and erector contractors and general contractors must require the installation of a center row of bolted bridging to provide lateral stability during construction prior to slacking of the hoisting line were long span joists or trusses are used (29 CFR 1926.751(c)(2)).
- The general contractor must ensure that a safety program/safety plan is established and implemented to prevent accidents. The safety program or plan must include frequent and regular inspections of job sites, materials, and equipment by a competent person (29 CFR 1926.20(b)(1)).
- General contractors must have all workers on the job site instructed in the recognition and avoidance of unsafe construction site hazards with their work and work areas (29 CFR 1926.21(b)(2)).
