Bulldozer Operator Dies in Construction Accident
A bulldozer operator was involved in site preparation when a hydraulic line ruptured. Hydraulic fluid sprayed into the engine and ignited. Flames shot out and engulfed the operator. He received severe burns. He died from his injuries.
OSHA Issues Two Serious Citations:
- Frequent and regular inspections of the equipment were not made by a competent person as required by 29 CFR 1926(b)(2).
- Workers conducting the inspections of the bulldozer were not trained to examine the hoses for signs of wear and abrasions as required by 29 CFR 1926.21(b)(2).
Prevention of Other Construction Fatalities because of Improper Inspections
- Workers must be trained to recognize and avoid construction site hazards with the operation of machinery (29 CFR 1926.21(b)(2)).
- Contractors must have properly trained competent persons to perform periodic inspections of the job site, materials, equipment and machinery (29 CFR 1926.20(b)(1) and (b)(2).
- Contractors must establish and enforce a safety program/plan that prevents construction site accidents (29 CFR 1926.20(b)(1) and (b)(2).
