I-Beam Falls from Height Injuring Worker

Construction workers were stacking I-beams in preparation for the structural steel erection. A laborer was placing a spacer between I-beams when the forklift operator was transporting an unsecured I-beam to be added to the stack. The I-beam fell from the forklift trapping the laborer between the falling I-beam and the stack.

Prevention of Construction Fatalities from Improper Work Practices

General Contractors and other contractors on the job site must:

  1. Instruct each worker in the recognition and avoidance of construction site hazards and OSHA regulations applicable to their work and work environment (29 CFR 1926.21(b)(2)).
  2. Control or eliminate any construction site hazards at the job site (29 CFR 1926.21(b)(2)).
  3. Ensure that workers have proper personal protection equipment and is used during all operations where there is an exposure to unsafe conditions (29 CFR 1926.28(a)).
  4. Ensure that forklifts carry stable and secure loads (29 CFR 1926.602(c)(1)(vi)).
  5. Ensure that a safety program/plan is established to prevent accidents. The program or plan must include frequent and regular inspections of job sites, materials and equipment by a competent person (29 CFR 1926.20(b)(1) and (b)(2)).
