Painter Death Preventable If He Had Worn the Proper Personal Fall Protection Equipment

Facts of Construction Accident Indiana
A painter was working inside of a storage silo. The interior of the silo is accessed by a fixed ladder system. The painter did not use the ladder, but used a makeshift boatswain's chair. While being lifted out of the silo, the bolts attaching the makeshift boatswain's chair with the winch line pulled out. The painter fell 48 feet to the bottom of the silo. He received a blunt deceleration trauma and died. If a proper personal fall protection equipment had been used, he would not have died.
Indiana State FACE Investigators' Recommendations to Prevent Similar Construction Fatalities
- Ensure the boatswain's chair seat used to sit on while painting the inside of the storage silo is of at least minimum dimensions, reinforced by cleats, and properly suspended.
- Employers should perform a hazard evaluation at each work site before any work is initiated.
- Employers should train employees in the recognition of hazardous work conditions and provide methods to control such hazards, including the proper use of appropriate fall protection equipment.
