Forklift truck operator was unloading a truck. The forklift truck came too close to the outer edge of the loading dock. The loading dock crumbled, and the forklift truck tipped over. When the operator falls from height, he became pinned underneath the forklift truck. He died from a traumatic asphyxia due to the workplace accident.
