Forklift truck operator was unloading a truck. The forklift truck came too close to the outer edge of the loading dock. The loading dock crumbled, and the forklift truck tipped over. When the operator falls from height, he became pinned underneath the forklift truck. He died from a traumatic asphyxia due to the workplace accident.

  1. Employers should ensure passageways are kept clean and in good repair where mechanical equipment is used.
  2. Employers should ensure employees receive the proper lift truck operator safety training.
