An Indiana Mechanical Designer Falls through Roof Opening and Dies

Facts of Construction Accident Indiana
A mechanical designer was on a roof alone. He was taking measurements for a new air vent system. While on the roof, he was unaware of a temporary roof opening. He stepped into the roof opening and fell 22 feet. He hit a crane before landing on the concrete floor. He received a blunt impact traumatic brain injury along with injuries to his chest, abdomen and extremities. He died as a result of these injuries.
Indiana State FACE Investigators' Recommendations to Prevent Similar Construction Fatalities
- Employers should ensure temporary roof openings are covered or guarded by a standard railing.
- Employers should ensure that appropriate fall protection is available and correctly used when working where there is danger of falling.
- Scheduled and unscheduled safety inspections should be conducted by a competent person.
