Electrocution Accident While Changing a Fuse

Facts of Construction Accident Indiana
An Indiana worker at a feed and grain mill received electrocution injuries while changing a fuse. The worker was loading a truck with fertilizer when the auger and conveyor belt stopped. He went to check the fuse box. The fuse box was not de-energized. While changing the fuse, he received a fatal electrical shock. He died from asphyxia due to electrocution.
Indiana State FACE Investigators' Recommendations to Prevent Similar Construction Fatalities
- Employers should conduct a jobsite survey before starting any work to identify any hazards, implement appropriate control measures and provide subsequent training to employees specific to all identified hazards.
- Employers should develop, implement and enforce a comprehensive written safety program.
- Employers should provide additional electrical safety training to those workers working with or around electrical current, including proper rescue procedures.
