Holes in walking/working surfaces are made during construction for duct work, skylights, etc. When these holes exist at a construction site, OSHA regulations mandate that contractors protect against falls from height by providing personal fall arrest systems, covers or guardrail systems. 29 CFR 1926.500 through 1926.503.
