Indiana Worker Dies from Defective Boatswain's Chair

Indiana worker died while making repairs to a storage silo. The storage silo had a fixed ladder system to access the inside. The fixed ladder did not allow the worker to perform the work necessary for the repairs. The worker's employer provided a makeshift boatswain's chair. The boatswain's chair failed when the bolts pulled out that held the chair to the winch line. The worker fell 48 feet to the bottom of the silo. He received a fatal severe traumatic brain injury.
Indiana investigator's recommended the following to prevent similar construction fatalities:
- Boatswain's chair seats should be reinforced by cleats and be properly suspended;
- Contractors should perform a hazard evaluation at each work site before any work is initiated; and
- Contractors should train workers in the recognition of hazardous work conditions and provide methods to control such hazards, including the proper use of appropriate fall protection equipment.
